Asthma in a chemist due to TBTU (new cause)

Asthma in a chemist due to TBTU (new cause)
Agent

TBTU (2-[1H-benzotriazol-1-yl]-1,1,3,3-tetramethyluronium tetrafluoroborate)

Job chemist in peptide laboratory
Specific IgE negative with TBTU/albumin conjugate with immunoCAP
Skin prick test positive 1mg/ml (also to HBTU)
Non-specific reactivity Histamine 26.7mg/ml post lactose challenge (normal) and 1.84 mg/ml post TBTU
Serial Peak Flow not done
Induced sputum Eosinophils 0.3% pre and 6.3% post TBTU challenge
Specific Challenge

Immediate asthmatic reaction to TBTU undiluted tipped for 60 minutes with negative lactose control

Follow-up

improved with RPE, enclosure and ventilation
